Welcome to the Health and Social Care section of the website. Shropshire County Council's Social Care Services are responsible for delivering a range of health and social care services for vulnerable people living in Bridgnorth district. Information is available about the community care and support services, which are available for people who need help because of frailty, illness, disability, mental illness or substance misuse. |

You will find information about how to ask for support, the process to be followed and the standards Shropshire County Council have set for delivering the following services.
- Adults and carers services help vulnerable adults and older people by providing services such as home and day care. Help is also available for those who need to live in a supported environment such as a residential or nursing home. They also provide help for people with mental health problems, a physical disability or a sensory impairment.
- The Adults with learning disabilities team work with people aged over 18, and their carers, to help them keep living with independence so that people can continue to make day to day choices about how they live.
- Children and Family Services offer support for children who are vulnerable or at risk because of a disbility, family circumstances or behavioural difficulties as well as information about placing children with foster carers or adoptors.
